Overview

Evofosfamide (TH-302) is a hypoxia-activated prodrug designed to selectively target the hypoxic regions of solid tumors. Under low oxygen conditions, it releases a cytotoxic DNA alkylator, bromo-isophosphoramide mustard (Br-IPM), which induces cell death in tumor cells while sparing normal tissues. Pemetrexed is an antifolate chemotherapeutic agent that inhibits multiple enzymes involved in folate metabolism and DNA synthesis. The combination of TH-302 and pemetrexed has been investigated for enhanced antitumor efficacy, particularly in non-squamous non-small cell lung cancer (NSCLC) and other solid tumors. Preclinical studies demonstrated that TH-302 enhances the activity of pemetrexed and other chemotherapeutics by targeting tumor hypoxia, with clinical trials evaluating safety, tolerability, pharmacokinetics, and preliminary efficacy[1][2][3][6].
