This investigational product is a **combination therapy** comprising three drugs: - **Evofosfamide** is a hypoxia-activated prodrug designed to release a cytotoxic alkylating agent specifically in low-oxygen (hypoxic) tumor environments, potentially enhancing cancer cell kill while sparing normal tissues. - **Zalifrelimab** is a monoclonal antibody that targets **cytotoxic T-lymphocyte antigen 4 (CTLA-4)**, functioning as an immune checkpoint inhibitor to enhance T-cell mediated anti-tumor activity. - **Balstilimab** is a monoclonal antibody targeting the **programmed cell death protein 1 (PD-1)** receptor, blocking PD-1 mediated immune evasion by tumors, allowing for increased immune system attack on cancer cells. This triplet is under clinical investigation as a **synergistic combination**, with evofosfamide disrupting hypoxia-mediated immunosuppression in the tumor microenvironment and the checkpoint inhibitors enabling immune recognition and clearance of otherwise resistant solid tumors. It is being explored primarily in advanced or metastatic castration-resistant prostate cancer, pancreatic cancer, and human papilloma virus (HPV)-negative squamous cell carcinoma of the head and neck (SCCHN)[3][6].
