A **combination therapy** of evolocumab and a statin is used to lower low-density lipoprotein cholesterol (LDL-C) more effectively than statin monotherapy. **Evolocumab** is a fully human monoclonal antibody that inhibits proprotein convertase subtilisin/kexin type 9 (PCSK9), leading to increased LDL receptor recycling and enhanced clearance of LDL-C from the bloodstream. Statins are small-molecule inhibitors of HMG-CoA reductase, reducing the synthesis of cholesterol in the liver and upregulating LDL receptors. The combination is mainly indicated in patients who require additional LDL-C lowering despite maximally tolerated statin therapy, particularly those with atherosclerotic cardiovascular disease, familial hypercholesterolemia, and mixed dyslipidemia. It is associated with significant risk reduction for major adverse cardiovascular events, has not been shown to increase risk for new-onset diabetes or severe adverse events, and is generally well tolerated[1][2][4][6].
