Exatecan mesylate + gemcitabine is a **combination chemotherapy regimen** evaluated for the treatment of advanced or metastatic pancreatic cancer. Exatecan mesylate (DX-8951f) is a potent, water-soluble, hexacyclic **topoisomerase-1 inhibitor** structurally related to irinotecan but is not a prodrug. It directly inhibits DNA topoisomerase-1, thereby interfering with DNA replication and causing tumor cell death. Gemcitabine is a nucleoside analog functioning as an antimetabolite, which incorporates into DNA during replication and causes chain termination, inhibiting tumor growth. The combination was developed due to **additive or synergistic effects** observed in preclinical models and their non-overlapping toxicity profiles. It was primarily tested as first-line treatment for advanced pancreatic adenocarcinoma in randomized phase III clinical studies. Although the combination showed antitumor efficacy and predictable toxicities (primarily hematologic effects), it did not significantly improve overall survival compared to gemcitabine monotherapy in clinical trials[1][4][3].
