This is a combination regimen consisting of three distinct drugs: - **Exemestane** is an oral steroidal aromatase inhibitor that irreversibly inhibits the aromatase enzyme, leading to decreased estrogen production. It is primarily used in hormone receptor-positive breast cancer, especially in postmenopausal women or premenopausal women with ovarian suppression. - **Leuprolide** is a gonadotropin-releasing hormone (GnRH) agonist that suppresses ovarian function by downregulating pituitary GnRH receptors, resulting in reduced secretion of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), and thus lowering estrogen levels. It is commonly used for ovarian suppression in premenopausal women with breast cancer. - **Pembrolizumab** is a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor on T cells. By blocking PD-1 interaction with its ligands PD-L1 and PD-L2, it enhances anti-tumor immune responses. Pembrolizumab has broad indications across multiple solid tumors. This combination may be considered for investigational use or off-label protocols where both hormonal therapy and immunotherapy are being explored together for synergistic effects—such as certain subtypes of advanced or recurrent breast cancer[4][5][6].
