Exenatide is a synthetic version of exendin-4, a 39-amino acid peptide originally isolated from the Gila monster. It acts as an agonist of the glucagon-like peptide 1 receptor (GLP-1R) and is used as an antidiabetic drug for type 2 diabetes. Exenatide stimulates insulin secretion in response to high blood glucose and is more resistant to DPP-4 degradation than native GLP-1[7][8]. Under the brand name Byetta, it is approved for use in type 2 diabetes. Exendin-(9–39) is a truncated form of exendin‐4 that functions as an inverse agonist or antagonist at the GLP‐1 receptor. It inhibits GLP‐1-induced insulin release and increases postprandial glucose levels[2][4]. Exendin-(9–39) has been studied primarily as a research tool and potential therapy for conditions such as congenital hyperinsulinism by blocking excessive insulin secretion[5]. The combination "exenatide + exendin (9–39)" refers to co-administration or study of both agents together; however, this combination does not represent an approved pharmaceutical product but rather two related peptides with opposing actions on the same target.
