This is a combination therapy consisting of three agents used in the management of type 2 diabetes mellitus: - **Exenatide** is a glucagon-like peptide-1 (GLP-1) receptor agonist that increases glucose-dependent insulin secretion, suppresses inappropriate glucagon secretion, and slows gastric emptying, leading to improved glycemic control and weight loss. - **Insulin detemir** is a long-acting basal insulin analog that provides steady background insulin levels to help regulate blood glucose throughout the day and night. - **Metformin** is a biguanide oral antihyperglycemic agent that decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. The combination targets multiple pathophysiological defects in type 2 diabetes. Clinical studies have shown that using exenatide with basal insulins like detemir on top of metformin can improve glycemic control (as measured by HbA1c), promote weight loss or minimize weight gain compared to basal-bolus regimens alone, reduce total daily insulin requirements, and may lower cardiovascular risk factors[1][6][7]. This regimen is typically considered for patients with inadequate glycemic control on dual therapy or those who are obese or at high cardiovascular risk.
