Exenatide + metformin is a combination therapy used in the management of type 2 diabetes mellitus. Exenatide is a glucagon-like peptide-1 (GLP-1) receptor agonist that increases insulin secretion, decreases glucagon secretion, and slows gastric emptying in response to food intake. Metformin is a biguanide antihyperglycemic agent that primarily decreases hepatic glucose production and improves insulin sensitivity. The combination is not available as a single fixed-dose product but is commonly prescribed together to improve glycemic control when monotherapy with either agent does not achieve adequate results. Clinical studies have shown that adding exenatide to ongoing metformin therapy leads to improved glycemic control without significant risk of hypoglycemia compared to some other combinations[1][3][6]. This regimen may also promote modest weight loss and has gastrointestinal side effects such as nausea and vomiting[2][3].
