Drug intelligence / Profile preview
exendin(4-39) is a synthetic peptide and an N-terminally truncated derivative of exendin-4 that acts as a glucagon-like peptide-1 (GLP-1) receptor antagonist. It is covered under a patent family originally assigned to Eiger BioPharmaceuticals (and subsequently acquired by Amylyx Pharmaceuticals) for the treatment of hyperinsulinemic hypoglycemia, including post-bariatric hypoglycemia and congenital hyperinsulinism. By removing the first three amino acids of the full-length exendin-4 agonist, exendin(4-39) retains antagonist activity at the GLP-1 receptor, similar to the more widely studied antagonist exendin(9-39) (avexitide).
