Expanded autologous adipose-derived stem cells (ASCs) is an investigational cell therapy developed by University Hospital, Toulouse for the treatment of non-revascularizable critical limb ischemia (CLI). The therapy involves the extraction of a patient's own adipose tissue, followed by the in vitro expansion of the mesenchymal stem/stromal cell population. These expanded cells are then delivered via multiple intramuscular injections into the ischemic limb. The mechanism of action is primarily paracrine, where the injected ASCs secrete a variety of angiogenic, anti-inflammatory, and anti-apoptotic factors that stimulate the formation of new blood vessels (angiogenesis) and promote tissue repair in the hypoxic environment. The safety and feasibility of this approach were evaluated in the ACellDREAM Phase I clinical trial, which showed improved wound healing and limb salvage in patients with no other revascularization options.
