Extended regimen (Mansoura University)

The Extended regimen (Mansoura University) is a preoperative antibiotic prophylaxis protocol developed for high-risk patients undergoing retrograde intrarenal surgery (RIRS) for renal stones. Evaluated in a Phase 3 clinical trial, the regimen primarily consists of a 7-day course of oral sulfamethoxazole-trimethoprim (TMP-SMX) administered twice daily, with the final dose given one day prior to the surgical intervention. In cases of patient allergy or documented bacterial resistance to TMP-SMX, the protocol utilizes alternative antibiotics such as nitrofurantoin, ciprofloxacin, or cefpodoxime. The regimen is designed to provide a prolonged duration of antimicrobial coverage to reduce the incidence of postoperative infectious complications, including systemic inflammatory response syndrome (SIRS) and urosepsis, which are significant risks in complex endourological procedures.
