Ezetimibe + statin and ezetimibe + fenofibrate are fixed-dose or co-administered combinations used to treat hyperlipidemia. Ezetimibe is a cholesterol absorption inhibitor that blocks the Niemann-Pick C1-Like 1 (NPC1L1) protein in the small intestine, reducing intestinal absorption of dietary and biliary cholesterol. Statins (such as simvastatin, atorvastatin, rosuvastatin) inhibit HMG-CoA reductase, decreasing endogenous cholesterol synthesis in the liver. Fenofibrate is a peroxisome proliferator-activated receptor alpha (PPAR-alpha) agonist that increases lipoprotein lipase activity, promoting triglyceride breakdown. The combinations provide complementary mechanisms for lowering LDL-cholesterol and triglycerides: ezetimibe plus statin targets both absorption and synthesis of cholesterol for greater LDL-C reduction than either alone; ezetimibe plus fenofibrate addresses both elevated LDL-C and high triglycerides in mixed dyslipidemia[2][5][6][8]. These regimens are indicated as adjuncts to diet for primary hyperlipidemia, heterozygous familial hypercholesterolemia, homozygous familial hypercholesterolemia (with other therapies), mixed hyperlipidemia (especially with fenofibrate), and sitosterolemia[7].
