famotidine + n-acetyl cysteine is an investigational combination therapy developed by Prisma Health-Upstate for the treatment of non-hospitalized adult outpatients with newly diagnosed COVID-19. The regimen combines famotidine, a histamine H2-receptor antagonist typically used for gastrointestinal conditions, with N-acetyl cysteine (NAC), a small molecule that serves as a precursor to the antioxidant glutathione and acts as a direct scavenger of reactive oxygen species. This combination was hypothesized to mitigate the inflammatory response and oxidative stress associated with SARS-CoV-2 infection, potentially preventing the progression to severe disease. A Phase I open-label clinical trial (NCT04545008) was initiated to evaluate the safety and toxicity of various dose combinations, but the study was ultimately terminated.
