FEC-T + pertuzumab is a neoadjuvant chemotherapy regimen specifically designed for the treatment of stage II-III HER2-positive breast cancer. This regimen was a primary arm in the TRAIN-2 phase 3 clinical trial, conducted by the Netherlands Cancer Institute. It follows a sequential administration schedule: three cycles of FEC (5-fluorouracil, epirubicin, and cyclophosphamide) followed by six cycles of paclitaxel and carboplatin. Throughout all chemotherapy cycles, dual HER2-targeted blockade is maintained using the monoclonal antibodies trastuzumab and pertuzumab. The regimen combines the cytotoxic effects of anthracyclines, alkylating agents, antimetabolites, taxanes, and platinum compounds with the targeted inhibition of the HER2 receptor. Trastuzumab and pertuzumab bind to distinct domains of the HER2 protein, effectively blocking its dimerization and downstream signaling pathways while also recruiting immune effector cells via antibody-dependent cellular cytotoxicity (ADCC).
