Overview

**Fecal microbial transplantation + nivolumab** is an investigational combination therapy involving the administration of fecal microbiota transplantation (FMT) from healthy or responder donors together with the monoclonal antibody nivolumab, a PD-1 immune checkpoint inhibitor. FMT introduces a diverse community of gut microbes into a patient's gastrointestinal tract, aiming to modulate the gut microbiome and enhance antitumor immunity. Nivolumab blocks the programmed cell death protein 1 (PD-1) receptor on T cells, releasing inhibitory signaling and promoting T-cell–mediated anti-tumor responses. The combination is primarily studied in advanced solid tumors, especially melanoma and non-small cell lung cancer, to overcome immunotherapy resistance by altering host microbial and immune environments. FMT is usually administered orally (capsules) or via colonoscopy, and nivolumab is administered intravenously. Clinical studies have shown that this combination may increase objective response rates and enable previously refractory patients to respond to immunotherapy while maintaining a favorable safety profile[3][5][7].
