Feladilimab + ipilimumab is a combination of two monoclonal antibodies intended as immunotherapy for cancer. Feladilimab is a humanized IgG4 monoclonal antibody that acts as an agonist of the inducible T cell co-stimulatory receptor (ICOS) on T cells, designed to enhance T cell function and promote antitumor immune response without depleting ICOS-expressing effector T cells. Ipilimumab is a human IgG1 monoclonal antibody that binds to cytotoxic T-lymphocyte antigen 4 (CTLA-4), blocking its inhibitory action and enabling a stronger T cell-mediated anti-tumor immune response. Combining ICOS stimulation and CTLA-4 inhibition was hypothesized to promote expansion and activity of cytotoxic T lymphocytes against tumors. Both agents are designed as cancer immunotherapies, with efforts primarily in solid tumors, especially head and neck squamous cell carcinoma (HNSCC) and other advanced/metastatic solid cancers. However, feladilimab development was discontinued after limited efficacy in mid-stage trials.
