fenofibrate + colesevelam

Fenofibrate + colesevelam is a combination therapy used to manage mixed dyslipidemia and hypercholesterolemia, particularly in patients who require both LDL cholesterol and triglyceride lowering. Fenofibrate is a fibrate that activates peroxisome proliferator activated receptor alpha (PPARα), leading to increased lipolysis, activation of lipoprotein lipase, reduced apoprotein C–III, and overall improved lipid metabolism. Colesevelam is a bile acid sequestrant that binds bile acids in the intestine, preventing their reabsorption; this depletes the bile acid pool and upregulates hepatic cholesterol 7-alpha-hydroxylase, increasing conversion of cholesterol to bile acids and upregulating LDL receptors for enhanced clearance of LDL-cholesterol from blood. The combination has been shown to produce greater reductions in LDL-C than either agent alone[1][5][8]. It is primarily indicated for mixed hyperlipidemia or primary hypercholesterolemia.
