**Fenofibrate + metformin** is an oral fixed-dose combination investigated for patients with **type 2 diabetes mellitus and dyslipidemia**, metabolic syndrome, and obesity-associated insulin resistance. It combines fenofibrate, a prodrug converted to fenofibric acid that activates peroxisome proliferator-activated receptor alpha to improve triglyceride-rich lipoprotein metabolism, with metformin, an antihyperglycemic biguanide that reduces hepatic glucose production and improves insulin sensitivity through mitochondrial energy-stress and AMP-activated protein kinase-associated mechanisms. Solvay sponsored clinical studies of fixed-dose formulations, including combinations containing fenofibrate 80 mg twice daily and metformin 1,000 mg twice daily. Clinical studies reported improvements in lipid, glycemic, inflammatory, hemostatic, and insulin-resistance measures; preclinical rat work also suggested protection from myocardial ischemia-reperfusion injury. This exact combination should not be assumed to be an approved or currently marketed single product in every jurisdiction.
