Fenofibrate + rimonabant is a combination of two small molecule drugs, each with distinct mechanisms of action, investigated primarily for the management of metabolic syndrome and related cardiometabolic risk factors in overweight or obese patients with dyslipidemia. Fenofibrate is a peroxisome proliferator-activated receptor alpha (PPARα) agonist that lowers triglycerides and increases HDL cholesterol by modulating lipid metabolism. Rimonabant is a selective cannabinoid type 1 (CB1) receptor inverse agonist that was developed as an anti-obesity agent; it reduces appetite and improves metabolic parameters by antagonizing endocannabinoid signaling in the central nervous system. Clinical studies have shown that their combination can lead to improvements in adipokine profiles, reductions in leptin and visfatin levels, and more pronounced reversal of metabolic syndrome criteria compared to monotherapy[2][3][5]. However, rimonabant has been withdrawn from the market due to psychiatric side effects.
