Overview

This is a combination of two potent synthetic opioid analgesics, fentanyl and remifentanil. Fentanyl is known for its rapid onset and relatively short duration of action compared to other opioids, but it can accumulate with prolonged use due to hepatic metabolism[7][8]. Remifentanil is distinguished by its ultra-short duration of action because it is rapidly metabolized by nonspecific blood and tissue esterases, resulting in a context-insensitive half-life of less than 10 minutes regardless of infusion duration[1][2][3][6]. The combination may be used to achieve rapid titration for varying levels of surgical stimuli or sedation requirements, offering flexibility in anesthetic management. Both agents share similar side effect profiles including respiratory depression, bradycardia, hypotension, and potential for dependence or abuse[2][7]. There are no unique brand names or fixed-dose products combining these two agents; their co-administration would be determined by clinical need.
