A combination therapy consisting of **ferric pyrophosphate citrate** (Triferic), a water-soluble iron salt, and **Shohl's solution** (a mixture of sodium citrate and citric acid, e.g., Oracit). This combination is under investigation for the treatment of **iron-refractory iron-deficiency anemia (IRIDA)**. Ferric pyrophosphate citrate directly donates iron to transferrin, bypassing hepcidin-induced block of iron export, while Shohl’s solution increases intestinal paracellular absorption by altering tight junctions, potentially enhancing iron bioavailability in IRIDA. Triferic is approved for iron maintenance in hemodialysis patients (administered via dialysate or IV), but its oral use with Shohl’s solution is investigational, studied in phase 1 clinical trials for efficacy and safety in IRIDA.
