iron sulfate + iron polymaltose refers to a combination of two distinct iron-replacement modalities: ferrous sulfate, a traditional divalent iron salt (Fe2+), and iron polymaltose complex (IPC), a non-ionic macromolecular complex of iron(III) hydroxide and polymaltose. Ferrous sulfate is characterized by rapid absorption via the divalent metal transporter 1 (DMT1) pathway, though it is frequently associated with gastrointestinal side effects due to the release of free ionic iron. In contrast, IPC mimics the structure of physiological ferritin, allowing for a more controlled, active transport-mediated absorption that significantly reduces oxidative stress and improves patient compliance. This combination is primarily utilized for the treatment of iron deficiency anemia, aiming to leverage the rapid hematological response of ferrous salts alongside the superior tolerability profile of iron complexes.
