Fespixon (ON101) is a first-in-class topical botanical drug developed by Oneness Biotech for the treatment of chronic diabetic foot ulcers (DFU). It is a biologic product formulated as a cream containing extracts from two medicinal plants, *Plectranthus amboinicus* and *Centella asiatica*. The drug's mechanism of action involves modulating the local immune environment of the wound by regulating macrophage polarization. Specifically, it promotes the transition from pro-inflammatory M1 macrophages to pro-regenerative M2 macrophages, thereby resolving chronic inflammation and facilitating tissue repair. Fespixon received regulatory approval from the Taiwan Food and Drug Administration in 2021 after Phase 3 clinical trials demonstrated a significantly higher complete healing rate compared to standard absorbent dressings.
