Fimaporfin + gemcitabine is a combination therapy developed for the treatment of cholangiocarcinoma, particularly in patients with inoperable or unresectable disease. Fimaporfin (also known as TPCS 2a or Amphinex) is a photosensitizer used in photochemical internalization (PCI), a technology that enhances the intracellular delivery of chemotherapeutic agents such as gemcitabine. Upon activation by light, fimaporfin disrupts endosomal membranes, allowing more effective release and cytosolic delivery of co-administered drugs like gemcitabine inside cancer cells. Gemcitabine itself is a nucleoside analog antimetabolite that inhibits DNA synthesis and induces apoptosis in rapidly dividing tumor cells. The combination has shown promising safety and efficacy results in early-phase clinical trials for cholangiocarcinoma, with improved response rates and survival compared to standard therapies[1][2][5].
