Overview

BR1400-4 is a specific dose strength of the fixed-dose combination (FDC) therapy BR1400, developed by Boryung Pharmaceutical for the treatment of essential hypertension. BR1400 consists of fimasartan, a non-peptide angiotensin II receptor antagonist (ARB), and chlorthalidone, a long-acting thiazide-like diuretic. Fimasartan selectively blocks the AT1 receptor, preventing the vasoconstrictive and aldosterone-secreting effects of angiotensin II. Chlorthalidone inhibits sodium and chloride reabsorption in the distal convoluted tubule of the kidney, leading to increased diuresis and a reduction in plasma volume. The combination is marketed in South Korea under the brand name Akarb and is indicated for patients whose blood pressure is not sufficiently controlled by fimasartan monotherapy. BR1400-4 specifically refers to the 120mg/25mg dose strength evaluated in Phase II clinical trials to optimize the efficacy and safety profile of the combination.
