Overview

Finafloxacin (AL-60371) is a novel fluoroquinolone antibiotic developed by Alcon (originally licensed from MerLion Pharmaceuticals) for the treatment of otic infections. The specific formulation identified as **AL-60371 + AL-817** refers to a single-application otic suspension where AL-817 is a thermosetting, poloxamer-based vehicle designed to provide sustained release of the antibiotic. Finafloxacin is distinguished from other fluoroquinolones by its enhanced antibacterial activity under acidic conditions, which are characteristic of the inflammatory environment in the infected ear canal. This single-dose formulation was primarily investigated for the treatment of acute otitis media with tympanostomy tubes (AOMT) to provide a more convenient alternative to multi-day dosing regimens. While a multi-dose 0.3% suspension of finafloxacin was approved by the FDA under the brand name Xtoro for acute otitis externa, the development of the single-dose AL-60371 + AL-817 formulation for AOMT was ultimately discontinued.
