Finafloxacin + esomeprazole is a combination of two pharmaceutical agents with distinct mechanisms and indications. Finafloxacin is a fluoroquinolone antibiotic that acts by inhibiting bacterial type II topoisomerase enzymes, specifically DNA gyrase and topoisomerase IV, which are essential for bacterial DNA replication, transcription, repair, and recombination. It is notable for its enhanced activity in acidic environments and is primarily used to treat acute otitis externa (swimmer's ear) caused by susceptible bacteria such as Pseudomonas aeruginosa and Staphylococcus aureus[6][7]. Esomeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by irreversibly binding to the Hydrogen/potassium ATPase enzyme (H+/K+ ATPase) on gastric parietal cells. This inhibition reduces both basal and stimulated acid production in the stomach. Esomeprazole is indicated for conditions involving excessive stomach acid such as gastroesophageal reflux disease (GERD), erosive esophagitis, peptic ulcers (including those associated with Helicobacter pylori infection), Zollinger-Ellison syndrome, and prevention of NSAID-induced ulcers[1][2][5][8].
