**Fish oil + soybean oil + medium-chain triglycerides + olive oil** is a four-oil intravenous lipid emulsion for parenteral nutrition marketed as **SMOFlipid** and developed by Fresenius Kabi. It is a sterile 20% lipid injectable emulsion composed of soybean oil, medium-chain triglycerides, olive oil, and fish oil, formulated to provide calories and essential fatty acids in patients whose oral or enteral nutrition is not possible, insufficient, or contraindicated. The product supplies a mixed fatty-acid profile that includes omega-6 essential fatty acids from soybean oil, rapidly metabolized medium-chain triglycerides for energy, monounsaturated fatty acids from olive oil, and omega-3 long-chain polyunsaturated fatty acids such as EPA and DHA from fish oil. Rather than acting through a single receptor target, its therapeutic effect is nutritional replacement through systemic lipid delivery and modulation of fatty-acid substrate availability within lipid metabolism pathways. It is approved for use in adult and pediatric patients, including term and preterm neonates, as a component of parenteral nutrition.
