FL115 + BCG is an investigational combination therapy under clinical evaluation for the treatment of non-muscle invasive bladder cancer (NMIBC)[1]. FL115 is a novel small molecule agent, currently in Phase 1/2 dose-escalation and cohort expansion studies, and is being tested alone or in combination with Bacillus Calmette-Guérin (BCG), which is a live attenuated vaccine strain of Mycobacterium bovis widely used as intravesical immunotherapy for NMIBC[1]. The precise mechanism of FL115 has not been disclosed; thus, its molecular targets remain unspecified. BCG’s antitumor mechanism primarily involves stimulating local immune responses, notably the activation of macrophages, dendritic cells, CD4+ and CD8+ T cells, and production of antitumor cytokines within the bladder. The goal of the FL115 + BCG combination is to enhance anti-tumor efficacy over standard BCG monotherapy, potentially through additive or synergistic immune and direct anti-cancer effects[1].
