FLAG-Mitoxantrone + venetoclax is an intensive investigational combination regimen for acute myeloid leukemia (AML) that adds the oral BCL-2 inhibitor venetoclax to a FLAG backbone in which idarubicin is replaced by the anthracenedione mitoxantrone. Venetoclax is a small-molecule BH3 mimetic that selectively inhibits the anti-apoptotic protein BCL-2, promoting mitochondrial outer membrane permeabilization and apoptosis of AML blasts, while fludarabine and high-dose cytarabine act as antimetabolites that impair DNA synthesis/repair and mitoxantrone intercalates DNA and inhibits topoisomerase II to induce DNA damage.[3][4] This regimen is being explored as a salvage or induction strategy for relapsed/refractory or high-risk AML, leveraging venetoclax’s pro-apoptotic activity to deepen remissions achieved with intensive chemotherapy, though it remains an off-protocol, non-standardized variation of established FLAG-IDA + venetoclax and CLAG-M + venetoclax regimens.[3][4][7][9]
