Overview

Flecainide + metoprolol is a combination therapy used primarily for rhythm control in patients with atrial fibrillation and other supraventricular arrhythmias. Flecainide is a class 1C antiarrhythmic agent that works by blocking fast inward sodium channels during phase 0 of cardiac depolarization, prolonging the refractory period and stabilizing cardiac electrical activity. It also inhibits delayed rectifier potassium channels and ryanodine calcium channels, further affecting myocardial action potentials. Metoprolol is a beta-blocker that reduces heart rate and myocardial contractility by antagonizing beta1 adrenergic receptors in the heart. The combination improves rhythm control effectiveness in persistent symptomatic atrial fibrillation and increases tolerability compared to monotherapy[2][3][5]. This regimen requires careful monitoring due to additive effects on cardiac conduction, which can increase the risk of bradycardia or other conduction abnormalities[1][4].
