Flt3 ligand + CD40 agonist + doxorubicin is an investigational triplet immunochemotherapy regimen combining a recombinant Flt3 ligand (such as CDX-301) to expand and differentiate conventional dendritic cells, a CD40 agonist antibody (such as CDX-1140) to activate antigen-presenting cells, and pegylated liposomal doxorubicin chemotherapy to induce tumor cell death and antigen release. Preclinical work in murine triple-negative breast cancer (TNBC) models showed that this triplet enhances tumor control versus doublet combinations by increasing dendritic cell–mediated antigen presentation and CD8 T-cell priming, leading to improved antitumor immunity. A phase 1 pilot trial in metastatic or unresectable locally advanced TNBC (NCT05029999) is evaluating safety, recommended phase 2 dose, and immunologic and clinical activity of this regimen, with CDX-301 given for early cycles and CDX-1140 plus liposomal doxorubicin continued until progression or toxicity.
