Overview

Flucloxacillin + gentamicin is a combination of two antibiotics used primarily for the treatment and prophylaxis of serious bacterial infections, especially those caused by susceptible gram-positive organisms such as Staphylococcus aureus. Flucloxacillin is a beta-lactam antibiotic (penicillin class) that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. It is stable against many beta-lactamases. Gentamicin is an aminoglycoside antibiotic that binds irreversibly to the 30S subunit of the bacterial ribosome, inhibiting protein synthesis and resulting in bactericidal activity. The combination offers synergistic effects—flucloxacillin disrupts cell wall integrity while gentamicin inhibits protein synthesis—resulting in enhanced bactericidal action and inhibition of exotoxin production, which can be particularly important in toxin-mediated diseases like toxic shock syndrome[1][4]. This regimen may be considered as an alternative to clindamycin-based therapy for severe staphylococcal or streptococcal infections.
