This therapeutic regimen involves the administration of the antifungal medication fluconazole in combination with an Interleukin-23 (IL-23) inhibitor (such as risankizumab, guselkumab, or the IL-12/23 inhibitor ustekinumab) for the treatment of Crohn's disease. Developed by researchers at Weill Medical College of Cornell University, the approach is based on the hypothesis that targeting specific fungal mycobiota (like *Candida albicans*) with fluconazole can enhance the clinical response to IL-23 blockade. IL-23 is a pro-inflammatory cytokine that plays a key role in the pathogenesis of Crohn's disease by promoting Th17 cell responses. Fluconazole is a triazole antifungal that inhibits the fungal enzyme lanosterol 14-alpha-demethylase, thereby altering the gut mycobiome to potentially reduce fungal-driven inflammation.
