Drug intelligence / Profile preview
fludarabine + busulfan + antithymocyte globulin (Flu/Bu/ATG) is a widely used reduced-intensity conditioning (RIC) or reduced-toxicity myeloablative conditioning regimen administered to patients prior to undergoing allogeneic hematopoietic stem cell transplantation (allo-SCT). The regimen combines three distinct therapeutic agents: fludarabine, a purine antimetabolite that provides potent immunosuppression to facilitate donor engraftment; busulfan, an alkylating agent that exerts myeloablative and antineoplastic activity to eradicate residual malignant cells; and antithymocyte globulin (ATG), a polyclonal antibody preparation that selectively depletes T-lymphocytes to prevent graft-versus-host disease (GvHD) and graft rejection. This combination is commonly utilized in patients with hematologic malignancies, such as acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), and myelofibrosis, particularly in older or less fit patients who cannot tolerate standard high-intensity myeloablative conditioning.
