Conditioning Regimen E is a specific preparative regimen used in the context of hematopoietic stem cell transplantation (HSCT), specifically evaluated in the CIBMTR ACCELERATE platform trial. It consists of a combination of the chemotherapy agents fludarabine and cyclophosphamide, along with low-dose total body irradiation (TBI). Fludarabine is a purine antimetabolite that interferes with DNA synthesis, while cyclophosphamide is an alkylating agent that cross-links DNA. The addition of TBI provides further myeloablation and potent immunosuppression to facilitate donor engraftment and reduce the risk of graft rejection. This regimen is designed to prepare patients with hematologic malignancies for mismatched unrelated donor (MMUD) peripheral blood stem cell transplantation, often used in conjunction with post-transplant cyclophosphamide (PTCy) for graft-versus-host disease (GVHD) prophylaxis.
