Overview

This combination is a complex myeloablative conditioning (MAC) regimen utilized in the context of haploidentical-cord blood hematopoietic stem cell transplantation (haplo-cord HSCT). It consists of the purine analog fludarabine, the antimetabolite cytarabine, the alkylating agents busulfan, melphalan, and cyclophosphamide, and the immunosuppressive antithymocyte globulin (ATG). Developed by Fujian Medical University Union Hospital, the regimen is specifically designed for patients with acute myeloid leukemia (AML) and high-risk myelodysplastic syndromes (MDS). The protocol employs a sequential infusion strategy where cyclophosphamide is administered within a critical window to balance myeloablative intensity and immunomodulation, facilitating the engraftment of both haploidentical stem cells and umbilical cord blood.
