The combination of fludarabine, cytarabine, and granulocyte colony-stimulating factor is known as the FLAG regimen. It is primarily used as a chemotherapy protocol for the treatment of acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS), especially in patients who are ineligible for anthracycline-based therapy due to age or comorbidities[2][4][5]. Fludarabine and cytarabine are antimetabolite chemotherapy agents that inhibit DNA synthesis, thereby preventing cancer cell proliferation. Granulocyte colony-stimulating factor (G-CSF) is a biologic growth factor that stimulates bone marrow to produce more neutrophils, aiding recovery from chemotherapy-induced neutropenia[4][5]. The regimen can be administered intravenously for the chemotherapeutics and subcutaneously for G-CSF. FLAG may be used as induction therapy or salvage therapy in relapsed/refractory AML or high-risk MDS[2][8].
