FLADx is a multi-agent combination chemotherapy regimen primarily utilized in the treatment of pediatric acute myeloid leukemia (AML). It consists of three active cytotoxic agents: fludarabine (a purine nucleoside antimetabolite), high-dose cytarabine (a pyrimidine nucleoside antimetabolite), and DaunoXome (a liposomal formulation of the anthracycline daunorubicin). The regimen is typically employed as an intensified induction therapy. In the NOPHO-DBH AML 2012 Phase III trial, FLADx was evaluated as a second induction course, compared against the ADxE regimen, to improve outcomes in pediatric patients. The treatment protocol often incorporates minimal residual disease (MRD) monitoring to guide risk stratification and subsequent therapeutic decisions, such as the allocation to allogeneic stem cell transplantation for high-risk patients.
