This is a reduced-intensity conditioning (RIC) regimen developed by the University of Chicago, specifically designed for patients undergoing combined haploidentical and umbilical cord blood stem cell transplantation (Haplo-Cord). The regimen consists of fludarabine (a purine analog chemotherapy), melphalan (a nitrogen mustard alkylating agent), and rabbit antithymocyte globulin (r-ATG, often administered as Thymoglobulin). Fludarabine and melphalan provide cytoreduction and myelosuppression to facilitate donor cell engraftment, while r-ATG provides potent T-cell depletion to prevent graft-versus-host disease (GVHD) and reduce the risk of graft rejection. This protocol is utilized for a wide range of hematologic conditions, including acute and chronic leukemias, lymphomas, multiple myeloma, myelodysplastic syndromes, and non-malignant disorders like aplastic anemia and hemoglobinopathies in both adult and pediatric populations.
