Overview

Fludarabine + Thymoglobulin is a combination regimen used primarily as part of conditioning protocols before allogeneic hematopoietic stem cell transplantation (allo-HSCT), especially in patients with severe aplastic anemia and other hematologic disorders. Fludarabine is a purine analog that inhibits DNA synthesis by interfering with DNA polymerase and ribonucleotide reductase, leading to immunosuppression and cytotoxicity against lymphocytes. Thymoglobulin (anti-thymocyte globulin) is a polyclonal antibody preparation derived from rabbits immunized with human thymocytes; it depletes T lymphocytes via complement-mediated lysis and apoptosis, providing potent immunosuppression to prevent graft rejection. The combination enhances engraftment rates by reducing host immune responses while minimizing toxicity compared to traditional regimens[2][3][4].
