Overview

**Fludarabine + Xcellerated T Cells** is a combination therapy involving the use of the chemotherapeutic agent **fludarabine** followed by the infusion of **Xcellerated T Cells**, which are autologous T cells expanded and activated ex vivo using anti-CD3 and anti-CD28 antibody-coated beads (the Xcellerate™ Process)[2]. Fludarabine is administered as a lymphodepleting agent to reduce native lymphocytes, creating a favorable environment for the infused T cells to engraft, expand, and exert anti-tumor activity. Xcellerated T Cells are intended to accelerate immune reconstitution, restore a diverse T cell receptor repertoire, and potentially induce anti-tumor effects. This combination has been tested particularly in patients with relapsed or refractory multiple myeloma, with the goal of improving immune recovery after autologous stem cell transplantation[2]. Fludarabine is a purine analog that inhibits DNA synthesis and induces cell death, while Xcellerated T Cells provide adoptive cellular immunotherapy. **Mechanism of action:** Fludarabine causes transient lymphodepletion and may also promote a shift toward a more responsive and mature immune milieu for subsequent T cell activation[1][2]. The Xcellerated T Cells, upon infusion, repopulate and enhance immune functions, particularly targeting tumor cells through direct cytotoxicity and cytokine secretion[2].
