Overview

Ftortiazinon + Maxipim is a combination therapeutic regimen consisting of fluorothiazinone, a first-in-class anti-virulence small molecule, and cefepime, a fourth-generation cephalosporin antibiotic. Fluorothiazinone targets the bacterial Type III Secretion System (T3SS), a highly conserved protein apparatus used by Gram-negative pathogens such as *Pseudomonas aeruginosa* to deliver effector toxins directly into host cells. Unlike traditional antibiotics, fluorothiazinone does not directly kill bacteria or inhibit their growth, which is intended to reduce the selective pressure for the development of antimicrobial resistance. Cefepime serves as the bactericidal backbone of the combination, providing broad-spectrum activity against Gram-negative and Gram-positive organisms. Developed by the Gamaleya Research Institute of Epidemiology and Microbiology, this combination is primarily being investigated for the treatment of complicated urinary tract infections (cUTI) caused by multidrug-resistant *P. aeruginosa*.
