fluorouracil + calcipotriol is a topical combination therapy being investigated by Maastricht University Medical Center for the treatment of actinic keratosis. The therapy combines 5-fluorouracil (5-FU), a pyrimidine antimetabolite that inhibits thymidylate synthase to block DNA synthesis, with calcipotriol, a synthetic vitamin D3 analog that acts as a vitamin D receptor agonist. This combination works synergistically by inducing the expression of thymic stromal lymphopoietin (TSLP) in keratinocytes, which activates a CD4+ T cell-mediated immune response against the precancerous lesions. In the CALM Phase 4 clinical trial, this combination is applied topically for a significantly shorter duration (4 to 6 days) compared to the standard 4-week 5-FU monotherapy, aiming to maintain efficacy while improving patient tolerability and adherence.
