Fluorouracil + cisplatin is a combination chemotherapy regimen consisting of the antimetabolite fluorouracil (5-FU) and the platinum-based alkylating agent cisplatin. In the context of the Japan Clinical Oncology Group (JCOG) 9907 trial, this regimen was evaluated as a neoadjuvant (preoperative) treatment for patients with stage II/III esophageal squamous cell carcinoma (ESCC). Fluorouracil acts by inhibiting thymidylate synthase, thereby interfering with DNA synthesis, while cisplatin forms DNA cross-links that impede DNA replication and transcription, leading to apoptosis. The JCOG9907 trial established neoadjuvant FP therapy followed by surgery as the standard of care in Japan for locally advanced ESCC after demonstrating superior overall survival compared to postoperative administration.
