**Fluorouracil + doxorubicin + streptozocin (FAS)** is a combination chemotherapy regimen used primarily for advanced pancreatic neuroendocrine tumors (PanNETs) and metastatic islet cell carcinomas. Fluorouracil inhibits thymidylate synthase to block DNA synthesis, doxorubicin intercalates DNA and inhibits topoisomerase II causing DNA damage, and streptozocin selectively alkylates DNA in beta cells while being less toxic to other tissues. Administered as fluorouracil 400 mg/m² IV days 1-5, streptozocin 400 mg/m² IV days 1-5, and doxorubicin 40 mg/m² IV day 1 every 28 days, it achieves objective response rates of 39-41% with median progression-free survival of 20 months and overall survival of 63 months in large retrospective cohorts of metastatic PanNETs.[2][4][5]
