Overview

This is a combination chemotherapy regimen known as FILM, consisting of four agents: fluorouracil (5-FU), ifosfamide, leucovorin (folinic acid), and mitomycin C. Each component has a distinct mechanism of action: - Fluorouracil is an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. - Ifosfamide is an alkylating agent that crosslinks DNA strands, leading to cell death. - Leucovorin enhances the binding of 5-FU to thymidylate synthase and helps reduce toxicity. - Mitomycin C acts as an alkylating agent causing DNA crosslinking. The FILM regimen has been studied primarily for advanced or metastatic breast cancer and inoperable or locally advanced disease. It has demonstrated high response rates with manageable toxicity profiles. The combination leverages different mechanisms to maximize cytotoxicity against rapidly dividing tumor cells[1].
