The combination of fluorouracil, leucovorin, oxaliplatin, epirubicin, and etoposide, commonly known as the EOELF regimen, is an intensive chemotherapy protocol developed by the Research Institute of General Surgery at Jinling Hospital for the treatment of advanced gastric cancer. This five-drug regimen integrates multiple cytotoxic mechanisms to maximize therapeutic efficacy: epirubicin (an anthracycline) and etoposide (a podophyllotoxin derivative) inhibit topoisomerase II and induce DNA damage; oxaliplatin (a third-generation platinum agent) forms bulky DNA adducts and cross-links; 5-fluorouracil (an antimetabolite) inhibits thymidylate synthase to disrupt DNA synthesis; and leucovorin (folinic acid) acts as a biochemical modulator that stabilizes the binding of 5-fluorouracil to its target enzyme. Primarily investigated in Phase II clinical trials, the EOELF regimen aims to improve response rates and survival in patients with metastatic or unresectable gastric malignancies.
