fluorouracil + oxaliplatin (Debiopharm)

This is an investigational chemotherapy regimen consisting of an intraoperative intraportal infusion of fluorouracil (5-FU) and oxaliplatin, followed by adjuvant mFOLFOX6 chemotherapy. Developed under the leadership of Xu Jianmin at Fudan University, the treatment is specifically evaluated in the Phase 4 Octree Study (NCT01972503) for patients with stage II or III colorectal cancer undergoing curative resection. The regimen aims to prevent liver metastasis by delivering high concentrations of chemotherapy directly into the portal vein during surgery to eliminate micrometastases. Fluorouracil is a pyrimidine analog antimetabolite that inhibits thymidylate synthase and incorporates into RNA and DNA, while oxaliplatin is a third-generation platinum-based agent that forms DNA adducts, leading to apoptosis and inhibition of DNA replication.
