This is a combination therapy consisting of two inhaled drugs—fluticasone furoate (an inhaled corticosteroid) and vilanterol (a long-acting beta2-adrenergic agonist)—together with oral voriconazole (a triazole antifungal). Fluticasone furoate reduces airway inflammation by acting as a glucocorticoid receptor agonist. Vilanterol relaxes bronchial smooth muscle via selective stimulation of beta2-adrenergic receptors, leading to bronchodilation. Voriconazole inhibits fungal ergosterol synthesis by blocking the cytochrome P450-dependent enzyme 14α-lanosterol demethylase, making it effective against Aspergillus species and other fungi. This specific triple regimen has been reported in case studies for the treatment of allergic bronchopulmonary aspergillosis (ABPA), particularly in patients where systemic corticosteroids are contraindicated or undesirable[1][8]. The combination leverages anti-inflammatory, bronchodilatory, and antifungal mechanisms to control both airway inflammation/obstruction and underlying fungal infection.
